Ways to increase the effectiveness of primary healthcare are currently being considered in Finland. Finnish institute for Health and Welfare, THL, develops measures of continuity of care. At the beginning of autumn, STM commissioned a report on the continuity of care model, which examined the continuity of the care relationship between the patient and the doctor. What are the regions’ experiences with service models that support continuity of care – is continuity of care best achieved with a personal doctor model, a team model or by increasing digital services?
